Effective Thursday April 24th, Syracuse Orthopedic Specialists will partner with MedRecs Management to handle all Disability and FMLA form requests. As a result, we will no longer accept Disability or FMLA forms in our offices. A limited selection of forms will still be accepted at our reception desk; this list will be available on our website. All other forms must be submitted through the new process.
You'll upload your form, provide details, and make a payment ($30 per form, $15 for updates, $50 for expedited requests).
Receiving your form:
You'll get an email or text with a secure link to download your completed form. You can also request it be faxed to a third party.

If you can't upload your form, ask a family member or friend for help or contact your local library for assistance.
